In my performance review this year, should I be honest with my boss?

You need to speak up or the situation will continue to get worse. For sure he is not going to fire you so lay it on the line exactly as you have in this question. He probably is unaware of the true situation. He is supervising a lot of people so he has to pay attention to the complainers. He is just uming that the others are happy and contented. A boss has to deal with the emergencies at hand.
